Services Dr. Andy provides for a quarterly fee: |

Patients have 24-hour access to Dr. Andy by cell phone, pager, or e-mail. |

Dr. Andy will make house calls to your home, office, or hospital |

Same-day appointments for illness or injury. No more waiting in “sick rooms” or ERs. |

• |
Evaluation of lung disorders, strep-throat, flu, urinary tract infections, stitches, wound care, more. |

• |
Comprehensive physicals and well-child visits, including EKG & lung function testing
(Expanded testing available.) |
| |
| |
• |
School & sports physicals. Vision and hearing screening if needed. |
| • |
Developmental and behavioral screening. |
| |
• |
Screening for skin cancer, cholesterol and other heart-related problems. |
| |
• |
Testing for prostate cancer, colon cancer, and treatment of other chronic medical problems. |
| |
• |
Dr. Andy has hospital privileges in Lee County for adults and
pediatrics. |
| |
• |
Active coordination of care with specialists. |
| |
• |
Dietary, nutrition, and exercise counseling. |
| |
• |
Travel medicine consultations, including emergency prescriptions and vaccine recommendations. |

• |
Referrals to respected physicians for your special medical needs. |
| |
• |
Supportive of alternative vaccination schedules if desired. |

| Quarterly Fee Schedule: |

Dr. Andy's patients are billed four times yearly, at the beginning of each quarter-year. |

ALL services provided by Dr. Andy are included in this quarterly fee. |

There are no additional per-visit fees, with the exception of vaccinations. |
| |

Dr. Andy is a "House Calls Only" doctor, who comes to YOU! The only one in SW Florida. |

• |
Fees are partially based on your location. |

Seasonal patients are charged for a minimum of two quarters per year, plus a 20% service charge. |
